HR strap disconnected mid activity

On two separate occasions my Polar Verity Sense got disconnected in the middle of activity and never reconnected. When I ended the activity Polar strap worked, but when I checked activities in SA there was missing HR data. Attaching screenshots.

Any ideas? That never happened before, started happening this year
